CIS form due by first day of school

It is important for you to keep your child’s immunization record up to date and report any changes to their form at school. We are required by the health department to collect immunization records of all of our students by the first day of school. Washington Law requires that the immunization form needs to medically verified. This means that unless the CIS record is printed by your doctor, any handwritten information you write on the CIS needs to be signed and verified by your doctor. You can also sign into myir.net and print one from home without it being verified.
